New sports physical forms

    Houlton High School has a new pre-participation physical evaluation form for student-athletes that allows for a physical every other year. The evaluation would be more extensive than the past required sports physicals.
    If a student-athlete needs a physical this year, the form must be filled out and recorded.    A parent needs to update a parents’ form each year. The forms may be picked up outside of the school office.
