PRESQUE ISLE, Maine — The Aroostook Health Network will host a live demonstration of the new online Pathfinder Case Management tool, the first of its kind in the state, at 12 p.m. on Thursday, Sept. 26, at Northern Maine Community College’s Edmunds Conference Center.
Members of the Aroostook Health Network, comprised of several county organizations, will share the process of how the project came about, introduce key partners who made the project possible and share plans for upcoming events, as well as educational and networking opportunities at this launch event. The network will also distribute the 2019 Community Pathfinder Resource Guide.
The Aroostook Pathfinder portal will be live the day of the event and members of the Aroostook Health Network will begin processing new registrations immediately.
Representatives from each of the network organizations, as well as the network coordinator, key project partners and attendees from over a dozen non-profit organizations and government entities will be present at the event.
The Aroostook Health Network is a collaboration of several non-profit organizations seeking innovative ideas to improve the access to quality healthcare and social services in Aroostook County.
